If your therapist requires formal supervision, you will be given a Disclosure Form that will state your therapist is being supervised and you will be asked to sign the Disclosure Form. The supervisor will review your diagnosis and treatment plan. Clinical notes are also reviewed. Your file will be kept confidential by your therapist and the supervisor and only released in accordance with law. You may request an appointment with the supervisor. First discuss your wish to do so with your therapist. Then make an appointment with the supervisor through the Office Staff.
