Welcome to the Center for Therapeutic Services & Psychodiagnostics (CFTSAP). For your convenience, we have posted online the forms and information that you may need to review and/or sign.
For your first appointment, please bring your insurance card and your driver’s license. (If your insurance changes, please let us know and please bring your updated insurance card). You will need to complete a client registration form.
To start therapy, we will need your signature on consent forms. If you want us to release information or an evaluation to a third party, you will need to sign the Release of Information form.
